This tri-fold patient brochure on hypertension was developed by Volunteers in Medicine (VIM) San Diego for their patient population.  It includes a description of hypertension, causes and impact of hypertension, and way to control hypertension. 

Patient Education Sheet – the DASH Diet
This patient education sheet was developed by volunteers of the Maliheh Free Clinic to be given to patients.  The education materials are intended to convey the information clearly to readers of all English reading levels.  This particular education sheet is on the DASH (Dietary Approaches to Stop Hypertension) diet and covers what the DASH diet is, how it works and tips for changing eating habits.

Patient Education Sheet – Decreasing LDL (Bad Cholesterol)

This education brochure was developed by Turning Points clinic to be given to patients suffering from or impacted by asthma.  The brochure is intended to convey important information regarding asthma to reader of multiple English reading levels.  This brochure covers asthma facts, common triggers and treatment.

COPD Education and Management Materials

Patient Brochure – COPD
This patient care flow sheet was developed by Cowlitz Free Medical Clinic to be used in the clinic to track COPD patients across multiple visits.  The flow sheet is intended to be used by clinic staff, and includes reminders of topics to cover during each visit, diagnostic, exacerbation, referral and medication information, as well as a goal-setting section.
